St. Clare Living Community of Mora has an opening for a Full Time Wellness Associate. The CNA certified Wellness Associate specializes in Exercise Groups and Wellness Programming. The Wellness Coach serves as the liaison between Wellness, Therapy, and Nursing and is responsible for managing and maintaining residents’ wellness/restorative nursing programs.

WELLNESS ASSOCIATE JOB DESCRIPTION
The CNA certified Wellness Associate specializes in Exercise Groups and Wellness Programming. The Wellness Coach serves as the liaison between Wellness, Therapy, and Nursing and is responsible for managing and maintaining residents’ wellness/restorative nursing programs.
QUALIFICATIONS REQUIRED
Certified Nursing Assistant be in good standing on the State Nursing Assistant registry or appropriate entity
Must have good communication skills
Ability to lead small groups
Must be able to read, write and communicate in English
QUALIFICATIONS REQUIRED
Previous hospital or nursing home experience
CPR certified
ESSENTIAL FUNCTIONS OF THE JOB, DEPARTMENTAL EXPECTATIONS
Conducts exercise groups that meet the MDS criteria for restorative nursing, documents the care provided, and reports findings to the Restorative Nurse Manager.
Provides treatments to meet individual’s needs (physical, cultural, and psychosocial) as per residents Plan of Care.
Responds promptly to acute changes in resident’s physiological state and reports modifications or adjustments to the treatment plan within limits of the Plan of Care.
Communicates resident progress and response to treatment to Wellness Director, Therapy, and Restorative Nurse Manager.
Resident/family education: Plans for and provides resident, family and caregiver education, identifies learning needs, clearly communicates information, and documents response to education provided.
Is responsible for maintaining specialized equipment, such as alarms, cushions, w/c’s, and coordinates equipment use with Nursing and Therapy.
Participates in plans for resident admission/discharge, anticipates long term and short term resident needs, and collaborates with the interdisciplinary team and make recommendations and attends Care Conferences as required.
Professional Growth: Attends meetings, workshops, seminars and participates in committees as requested/required by facility.Based on observations, makes suggestions to supervising nurse for new exercise/restorative programs which might benefit the resident.
