Recuperate with quality care to ease the transition home.

Recover in our unit dedicated to rehabilitation in one of 14 private rooms with a private bath.  Dining and therapy services are close by.  We offer admissions seven days a week and coordinate closely with discharge planners. 

Once you are here, a team including an  admission coordinator, social worker, nurses, nursing assistants, therapists, dietitian, and activity staff work with rounding physicians and a certified nurse practitioner to ensure we are meeting your needs.  Support staff provide great food and a clean and safe environment.

In addition to a wide variety of skilled nursing services, we offer in-house therapy utilizing our physical, occupational and speech therapists.  Depending on your needs, services are offered through an occupational therapist with an emphasis in Gerontology and a certified Lymphedema therapist.

Once your individualized plan has been developed with your personal goals in mind, our team provides you with continuity of care to meet your goals and return to your safest level of activity.

We have a variety of options available that will assist you with your recovery, provide better pain control, improve range of motion, and increase tolerance for exercise, and we want to make sure your recovery continues once you leave us. We will assist you as you approach your discharge date to identify vendors, sources, and payment options for adaptive equipment or assistive devices that you may need.

We encourage returning for outpatient therapy to sustain outcomes and functional ability gained during your stay.  We can help you achieve your highest level of function and address concerns that may surface after you return home. 

We can help to coordinate home health care including nursing, personal care, or therapy services.

Promoting the physical, mental, social, intellectual, and spiritual well-being of each individual is important at St. Clare Living Community. Our activities staff provides a variety of programs seven days a week.  Recreation Calendars listing all programs offered are posted monthly and everyone is welcome and encouraged to participate.

The Activity Room has books, magazines, puzzles, games and other leisure supplies and can be reserved for special family/friend gatherings.

FAQs

How do you help me with discharge planning? 

A successful transition to home begins the day you arrive.  Your caregivers consist of nurses, therapists, a social worker, a dietician, and an activity coordinator.  They will meet with you to administer assessments and obtain information about your current situation, goals, and needs at home.  Staff will make recommendations for your health and safety.

How will I know when I am ready to go home?

Staff members work closely with your physician to establish treatment goals and to assist you when you are ready to discharge home.  Your social worker will help you coordinate assistance at home so that your transition is successful.

What can I expect when I get home?

Services at home may include home therapy, home health care, nursing care at home, and meals on wheels.  Outpatient therapy is available at Westview or your local clinic as ordered by your doctor.

What if I need equipment at home?  

Your therapy staff members work with you to identify the adaptive equipment and assistive devices that would make living at home successful and will assist in obtaining needed items.
